Bone marrow aspirate concentrate (BMAC) may offer a regenerative treatment option for carefully selected knee conditions. At Orchard Orthopedics, Dr. Thon uses BMAC selectively when the underlying problem and the patient's goals make a biologic approach reasonable.

What Is It?

What Is BMAC for Knee Conditions?

BMAC stands for bone marrow aspirate concentrate. It is prepared from a small sample of your own bone marrow, which is then processed to concentrate its cellular and signaling components.

BMAC contains a mixture of cells and other substances involved in the body's natural repair processes. It is different from PRP, which is prepared from your blood and concentrates platelets.

The goal of BMAC treatment is to support the body's healing environment around injured or degenerative tissue. It is not a guaranteed method for rebuilding cartilage, reversing arthritis, or avoiding surgery.

What To Expect

How BMAC May Help the Knee

Depending on the diagnosis, BMAC may be considered for selected patients with:

  • Knee osteoarthritis and degenerative joint changes
  • Certain cartilage-related conditions
  • Selected tendon or other soft-tissue problems around the knee
  • Persistent symptoms despite conservative treatment
  • Conditions where supporting the body's repair response may be reasonable

The evidence surrounding BMAC for knee conditions is still evolving. Results can vary considerably depending on the condition being treated, how the BMAC is prepared, how it is delivered, and the characteristics of the patient.

For that reason, BMAC should be viewed as a selective treatment rather than a universal solution for knee pain.

Am I a Candidate?

BMAC may be worth considering if:

  • You have a knee condition that may be appropriate for a biologic treatment
  • Your symptoms have continued despite physical therapy and other conservative measures
  • You want to explore a regenerative option before considering surgery
  • Your imaging shows a potential target for treatment
  • You understand that the goal may be pain relief and improved function rather than complete restoration of damaged tissue

BMAC may not be the right fit if:

  • You have advanced arthritis with severe joint destruction
  • Your knee requires a more definitive surgical solution
  • You have substantial instability or mechanical symptoms
  • You expect BMAC to completely regenerate severely worn cartilage
  • Your condition does not have a reasonable biological target for treatment

Dr. Thon will evaluate the condition of your knee and explain whether BMAC has a realistic role in your treatment.

FAQ

Frequently Asked Questions

Understanding BMAC

BMAC should not be presented as a guaranteed cartilage-regeneration treatment. It may support a healing response in selected conditions, but it cannot reliably restore severely damaged cartilage or reverse advanced arthritis.

It may provide symptom improvement for some patients, but results are variable and the evidence continues to evolve. The severity of your arthritis is an important factor in deciding whether BMAC is reasonable.

There is not a simple answer. PRP and BMAC are different treatments, and evidence varies by condition and treatment protocol. Dr. Thon will help determine which, if either, is appropriate for your knee.

It may be worth considering before knee replacement for some patients who are not yet at the point where replacement is necessary. However, BMAC cannot reliably reverse advanced arthritis or guarantee that you will not eventually need surgery.

The next step depends on the reason your knee still hurts. BMAC may be an option for selected conditions, but Dr. Thon may recommend another treatment or surgery if the underlying problem is unlikely to respond to regenerative treatment.

BMAC is intended to support a gradual biological response rather than provide immediate pain relief. Improvement, when it occurs, is generally assessed over several weeks or longer.

Your return to exercise depends on what was treated and how much protection the tissue needs. Dr. Thon will provide a progression appropriate to your condition and activity goals.

In many cases, BMAC can be considered as part of a nonsurgical treatment strategy before surgery. If it does not provide sufficient improvement, your treatment options can be reassessed.
